So far, only one very expensive remedy can get rid of liver transplant as a treatment for serious liver disease. The doctors of the Gastroenterology Department of Delhi’s Sir Gangaram Hospital have saved the life of a liver patient by treating them very cheaply and have also claimed to have discovered a new and cheaper treatment for this disease.

On further tests, he was found to be Hepatitis B virus positive and required immediate treatment for Chronic Liver Failure (ACLF). Dialysis was considered in this condition and liver transplant was suggested to the patient as all the investigations of the patient revealed that the patient’s survival chances for one month were about 50%.

The patient was discharged in good condition after 20 days of hospitalization and after a month’s follow-up, the water accumulation in the patient’s abdomen was completely gone and jaundice became normal.

Dr Ranjan said that plasmapheresis is increasingly being used in liver transplant patients and in many cases a large proportion of these patients can be cured by controlling other factors. For example, in our patient who was being treated for hepatitis B virus with medication, PLEX worked to support the deteriorating liver.

Plex is a process similar to hemodialysis where blood is removed from the patient and the cellular components (RBCs, WBCs and platelets) are separated from the plasma by centrifugation in a machine. The plasma is removed and fresh plasma (FFP) and albumin are mixed with the cellular components and returned to the patient. Plasma contains a lot of toxic products that damage the liver Since the whole plasma is removed in PLEX, it is a better solution than dialysis and MARS.

Dr Ranjan says the cost of PLEX is around Rs 30,000 for each dose and blood donors are needed. Patients who respond have an average of 3 sessions of PLEX and some may require more sessions as our patient required 5 sessions. Although it is very cheap compared to liver transplant.

We routinely offer PLEX to our patients for the most common indications of failure of steroid treatment for severe alcoholic hepatitis, hepatitis B, and autoimmune hepatitis. We select these patients based on well established criteria. We have operated 10 patients with good results and saved the need for high cost liver transplant which as on date costs around 25 to 30 lakhs in private sector.
